The main butt of this satire is how physicians have to compete over patients in its contemporary system of socialized medicine in Italy. It doesn't really have the wisdom, depth or emotional resonance that you see in prime commedia al'Italiana, and is scarcely funny relative to the sheer volume of words in the dialogue. To me it is an "ordinary" comedy akin to what you see in middling English-speaking 1960s scripts, though I do enjoy Alberto Sordi much better than Jerry Lewis and such.
