90s-2000s Mysterious Action film??

I watched it when I was younger on DVD, it begins in 1st person, the main character is being helped by the ambulance on-site in a car wreck, and he has a flash back that consist for the remainder of the movie except at the end, and the main character is kinda following around this slightly goth guy with a bowl cut hair cut, I vaguely remember. They go to a manatee aquarium and the goth guy remembers being there as a kid, and eventually the main character goes to this house with an old person inside and is talking to them later to find out that they died years ago and the house has been vacant. The end, they are carrying the main character out on a stretcher, and he looks over and sees the goth guy also in the car wreck. Very strange movie! It definitely shows a story rather then telling it. Much appreciated!
