World War II combat nurse Claire Randall is transported back to 1743 Scotland, where she is kidnapped by Highlanders and meets an injured man named Jamie.

Claire is taken to meet the Laird but suspicions about her start to mount as the clan discover her extraordinary medical skills. She is held prisoner, accused of being a English spy.

Claire's abilities as a healer puts her at odds with the town priest when she saves a boy's life, and deepens the suspicions against her. Hearing a folk-tale, Claire realizes that she may be able to travel back through the stones to Frank.

Claire is now confident enough to attempt an escape. Under the cover of playing with the children she's had the opportunity to explore the woods outside the castle and has planned a route to get back to the stones. It's also the gathering of the MacKenzie clan, which should provide good cover. What she doesn't count on is bumping into Jamie, who seems to be hiding out in a barn. They return to the castle where members of the clan swear fealty to the Laird, putting Jamie in an awkward position.

Claire goes on the road with Dougal as he collects rents from the tenants. She meets the clans solicitor who is riding with them. She also becomes aware of Dougal's involvement in trying to raise money for the Jacobite rebellion.

Claire and Dougal are questioned at the local garrison, and Dougal is treated with hostility and disdain by the British occupiers. Claire lets down her guard, but her headstrong opinions get her into trouble again, and she discovers the true nature of Black Jack Randall when left alone with him.
