It takes a lot for a movie to disturb me, for instance I didn't really flinch when in Gozu, after a long sequence involving a minotaur and a room filling up with breast milk, a woman pretty graphically gives birth to a pretty insane looking full-grown man.
I might find some documentaries disturbing, but not necessarily in the way you're talking about. Although, perhaps the documentary Blood of the Beasts might fall into that category as well.

3 most disturbing films I've seen:
1) Threads (1984) (A dramatization of the probable consequences of a full scale nuclear war on an English community.)
2) Night and Fog (1955) (One of the most vivid depictions of the horrors of Nazi Concentration Camps. Filmed in 1955 at the post-war site of Auschwitz...)
3) Funny Games (1997) (Two psychotic young men take a mother, father, and son hostage in their vacation cabin and force them to play sadistic "games" with one another for their own amusement.)
4) The Vanishing (1988) ==> the original (Rex and Saskia are on holiday, a young couple in love. They stop at a busy service station and Saskia disappears...) ==> creepy,disturbing character, but amazing performance from Bernard-Pierre Donnadieu.

1) Triumph of Will - From a film making standpoint it's an incredible film however I find it probably the most unsettling film I've ever seen because it makes Nazi's look amazing.
2) Straw Dogs - It's an incredible film that explores serious taboos but I've seen it once and I'll never watch it again. It was far too emotionally draining.
3) The Hills Have Eyes (Remake, haven't seen the original) - I don't really find the film itself disturbing but rather the fact that some view this as entertainment. It's shallow as hell and attempts to make up for that with glorified violence as the audience is supposed to relish in the torture of the protagonists. It's torture porn and I hate torture porn.
4) À l'intérieur (aka Inside) - Again this is just torture porn, the plot is fucking ridiculous and characters survive multiple mortal wounds. They don't go unscathed instead they are viciously scarred and maimed but just keep going. There's one good scare in the film, the rest is just torture and suffering as entertainment.
5) Cannibal Holocaust - Probably the least disturbing out of the most disturbing films I've seen if only because at the end even the movie itself admits that it's some really sick shit.

Having seen the other 4 on your list, I dl/ed this one out of curiosity. I thought I had a strong stomach and disregarded the reviews. OMFG! I was so disgusted by halfway and I couldn't stop the convulsions building up I had to quit. Sickness has no boundaries, this is even extreme for a snuff alike.

And have some of already seen all the movies from the "Disturbo 13"? I only know "The 120 Days Of Sodom" (one of my favorite movies, but it didn't disturbed me), "Eraserhead" (well, that was a nightmare, but it could be harder), "Henry - Portrait Of A Serial Killer" (A great thriller, but it's just thrilling and not disturbing) and "The Last House On The Left".

Eraserhead. For me, it was more disturbing than Cannibal Holocaust, The 120 Days Of Sodom or The House on the Left.
I'm a little embrassed to say that "Final Destination" was also disturbing, at least for the ones who have flying phobia such as me.

I just watched The House on the Edge of the Park and that is definitely one to add to my list. One scene I found particularly disturbing and easily the film's most infamous scene is the scene where David Hess' character strips Cindy naked, who is a virgin, and then slices into her breasts with a switchblade.
